Scantibodies Clinical Lab cares about you and your patients. We want to show our commitment by sharing some vital information and discoveries that have been made relative to the parathyroid hormone and PTH testing within the last few years. To that end, here are some key points of information:

  1. 7-84 PTH is now recognized as the 2nd PTH hormone. It is produced by the parathyroid gland, like 1-84 PTH, but has an inverse action to 1-84 PTH in the body.
  2. Using this 7-84 PTH hormone in a ratio of 1-84 PTH/7-84 PTH accurately predicts adynamic bone disease, which afflicts 50% of ESRD patients.

    1. Bone biopsy procedures have also demonstrated that a measure of intact PTH or a single 1-84 PTH alone CANNOT identify the majority of adynamic bone disease patients compared to the 1-84 PTH/7-84 PTH ratio, because they do not measure the hypocalcemic 7-84 hormone separately.

    1. 7-84 PTH has opposite biological actions compared to 1-84 PTH in that:
      1. 1-84 PTH raises bone turnover necessary for bone growth.
      2. 7-84 PTH lowers bone turnover
    2. A ratio of 1-84 PTH/7-84 PTH has been shown (through bone biopsies) to be 94% accurate in identifying patients with adynamic bone disease.

    1. For African-American ESRD patients, this is particularly problematic. Dr. Malluche’s group, using bone histology, has demonstrated that the intact PTH assay has no correlation with bone status in African American ESRD patients.

    1. The average level of intact PTH for the African-American with histologically-confirmed adynamic bone disease is the same as the average level of intact PTH for the Caucasian with histologically-confirmed high bone turnover disease. This sets the African-American ESRD patient at particular risk of overdosing with vitamin D, calcium supplements and calcimimetics, leading to adynamic bone disease and soft tissue calcification.
